I worked with someone with the last name Dick. I asked her after about 3 years why her husband went by his last name. He didn’t. His first name was Richard, and he had been called Dick from the time he was a kid, and “Double Dick” to his friends. Harry Dick was his uncle. I live in a town where Harry Baals, pronounced “balls”, was a good mayor in the 1950’s. A park has a street named for him; they gave up replacing the signs after constant theft. And his wife? She was “Minnie”. So a proper introduction would have been “Presenting the Honorable Mayor Harry Baals, and his lovely wife, Minnie Baals. Yikes.
